2020-2021 Session Registration Form

Please send one form per child.  Please fill out this PDF form, print and mail to the church, or save it and email it to Kari.

Fall Semester begins Wednesday, September 9, 2020

Time:                     9:00 a.m. until 2:30 p.m.

Registration Fee:      $50 / yr

Tuition:                   $190 / month (both days),  

                             $100 / month (Monday only), $120 / month (Wednesday only)

                             Pre-K (Ages 4-5): Both days encouraged for curriculum effectiveness

                             *Class is determined by the child’s age as of September 1, 2019*

Additional Child

Tuition Discount:      $20/month if attending both days, $10/month if attending one day

Supply Fees:            Due at the beginning of each semester

                             Infants: $30/semester              Toddlers to Pre-K: $60/semester

                             *Students attending one day a week will pay ½ of the supply fee.*
